Cherry Printers No Carbon Required (NCR) School Sickness/Injury Form Triplicate books are ideal for keeping a record of any pupils ailments or injuries whilst in school. At a handy size of A5 which is 210mm x 148mm. The top & middle copy is perforated for easy removal so a copy can be sent home to parent, one can be filed and the bottom copy remains in the book for your records. It is stitched and bound with tape on the top (long edge) for a traditional finish. With plenty of space and information to give a very detailed account of the sickness or injury incurred. The book has 50 triplicate sets with a loose leaf writing shield.
