Each form includes dedicated sections for the student's name, form group, exempted uniform items, reason for exemption, issue date, validity period and issuing staff member. The simple layout helps maintain clear records while ensuring students have written confirmation of approved uniform exceptions.
Manufactured using carbonless NCR paper, every completed form automatically creates a duplicate copy. The perforated top copy can be removed and issued to the student, while the duplicate remains securely bound within the book for school records.
Containing 50 duplicate sets in a practical A6 format, this exemption book is ideal for secondary schools, academies, colleges, sixth forms and educational establishments that require an efficient and professional uniform management system.
