Cherry Printers No Carbon Required (NCR) Notification of Allergies Duplicate book is a practical way of informing anyone caring for your child of their allergies.
The book has been created so can work for a parent to fill and give to childcare providers or can also be used by the childcare providers like school, day care & clubs.
As the book is Duplicate NCR, both parties get to keep a copy of the filled in form. The top copy is perforated for easy removal and the bottom copy remains in the book for reference. At a handy size of A5 which is 210mm x 148mm. It is stitched and bound with tape on the top (short edge) for a traditional finish. All books come as 50 sets with a loose leaf writing shield.
